How do I fill in the new Prescription Form 13 correctly?

At the start of the new year, the existing forms 13, 14 and 18 will be replaced thanks to a new prescription form (HMV) – form number 13. In this blog post, we would like to show you what to look out for when filling in the new prescription form and what you need to do if you receive incorrectly issued prescriptions.

Medication Prescription Form 13

Personal details (mandatory field):
You must give all personal details; otherwise, treatment cannot begin. Corrections and additions require the doctor’s signature again.
 
Exemption from co-payment and co-payment obligation (optional):
The prescribing doctor can give the answer with a cross, indicating whether an exemption is in place. If this is the case, ask the patient to show you their exemption card. No correction is necessary here.

Consequences of an accident (optional):
If the prescription relates to the consequences of an accident, this box must be ticked. However, no correction is necessary.

BVG (optional):
If the prescription is issued under the BVG, this box must be ticked. However, no correction is necessary.

Selection of medicinal products (optional):
The therapeutic area can be selected by ticking the box. If this information is missing, the prescribing doctor should be informed.

Treatment-relevant diagnosis(es) (mandatory field):
The treatment-relevant diagnosis must be provided in the form of one or more ICD-10 codes and/or as plain text. The treatment-relevant diagnosis must be in place by the time of billing to the health insurance provider at the latest.

Diagnosis group (mandatory field):
The diagnosis group in accordance with the therapeutic products catalogue is a mandatory field. Corrections and additions must be made via the prescribing doctor.

Leading symptoms (mandatory field):
The leading symptoms must be ticked as either a, b or c (in accordance with the Therapeutic Products Catalogue). If the leading symptoms are specific to the patient, free text must be given. Corrections are possible following consultation with the prescribing doctor. 

Medicines in accordance with the Medicines Catalogue (mandatory field):
This is a mandatory field. Corrections require the doctor’s signature again. If details are added, the doctor must be informed and the change widely documented on the reverse side.

Treatment sessions (mandatory field):
The number of treatment sessions given is largely based on the Remedies Catalogue. Information may only be added to or altered by the prescribing doctor.

Supplementary remedies (not applicable):
The doctor can prescribe them in accordance with the Remedies Guidelines.

Treatment report (optional):
If a cross has been set, the prescribing doctor requires a treatment report from you.

Home visit (optional):
You can only bill for a home visit if this field is ticked with ‘yes’.
 
Treatment frequency (mandatory field):
A treatment frequency or frequency range must be given. Guide values are provided in the Remedies Catalogue. Adjustments are possible following consultation with the prescribing doctor. If no frequency is given, the recommended frequency of every 4 to 6 weeks applies automatically.

Urgent need for treatment (optional):
Ticked if treatment must start within 14 calendar days; otherwise, treatment starts within 28 calendar days. The urgent need for treatment can only be revoked thanks to the doctor.

Where applicable, treatment objectives / further medical findings and notes (optional):
Optional details of the prescribing doctor. 17. Contract doctor’s stamp (mandatory field)

Service provider’s ID (mandatory field):
To be completed by you as the therapist.

Confirmation of receipt by the insured person (mandatory field):
Actions taken must be clearly widely documented on the day the service is provided, including the date and initials of the service provider. Any corrections must be signed by the insured person.

Billing details of the treatment provider (mandatory fields)

Termination of treatment (optional):
If treatment is terminated, the reason and date must be noted.

Service provider’s stamp/signature (mandatory field)

Despite the significant simplification of the prescription for therapeutic measures, incorrectly completed prescriptions will continue to occur in future.
